Epidemiological investigation and analysis of a tuberculosis cluster epidemic in a school in Qiandongnan Prefecture

Abstract:Objective To analyze a tuberculosis cluster epidemic in a middle school in Qiandongnan Prefecture, and explore the causes of the epidemic, so as to provide reference for the scientific development of school tuberculosis prevention and control measures. Methods The epidemiological data and screening results of close contact with the tuberculosis cluster epidemic in a middle school in Qiandongnan Prefecture from March to May 2022 were collected. Descriptive epidemiological methods were used to analyze the epidemiological characteristics, screening results of close contact, and causes of the epidemic. Results A total of 2 293 students and teachers were screened; and 13 cases of pulmonary tuberculosis were reported among the students. All the cases were concentrated in Class 3, Grade 1 of senior high school. The attack rate of this class was 24.53%, and the distribution of cases showed the phenomenon of class and dormitory clustering. The strong positive rate of close contacts was 64.15%, higher than that of the general contacts (χ2=41.04, P<0.01). The index cases were symptomatic presentations, and the remaining cases were found by active screening, and none of them had symptoms related to tuberculosis. Conclusion The main reasons for the cluster epidemic are that the school did not carry out morning and noon examinations as required, the physical examination institution did not carry out the physical examination of new students in accordance with the national physical examination standards, the lack of knowledge of tuberculosis prevention and treatment among students, the sick students brought sick to class, and the use of air conditioning in the class also increased the risk of tuberculosis transmission.
