Abstract:Objective To explore the clinical value of prenatal ultrasound screening for fetal congenital heart disease and analyze the high-risk factors. Methods The clinical data of 2 869 pregnant women who underwent prenatal fetal congenital heart disease ultrasound screening in our hospital from January 2019 to July 2020 were retrospectively analyzed to calculate the sensitivity, specificity and accuracy of ultrasound in the diagnosis of fetal congenital heart disease. The patients with fetal congenital heart disease were regarded as the congenital heart disease group, and the patients without fetal congenital heart disease were regarded as the normal group. Logistic regression was used to analyze the high risk factors of fetal congenital heart disease. Results Among 2 869 pregnant women, 126 cases had fetal congenital heart disease, the detection rate was 4.39%, the sensitivity of ultrasonography was 87.30% (110 / 126), the specificity was 88.52% (2428 / 2743), and the accuracy was 88.46% (2538 / 2869). Compared with the normal group, the NT value in the early stage of the fetal heart disease group was higher than that in the normal group or the cervical lymphatic cystic tumors (61.90%), their mothers suffered from connective tissue disease or diabetes mellitus (64.29%), with the family history of congenital heart disease (77.78%), and the abnormal amniotic fluid (62.70%) were higher than those of the normal group (35.29%, 46.85%, 60.30% and 44.29%). (χ2=36.850, 14.688, 15.480, 16.479, all P<0.001). Logistic regression analysis showed that the high NT value in the early stage or cervical lymphedema, mother‘s connective tissue disease or diabetes, family history of congenital heart disease and abnormal amniotic fluid were all high risk factors for fetal congenital heart diseases (P<0.001). Conclusion Prenatal ultrasound screening for fetal congenital heart disease is of high diagnostic value. High early fetal NT value or cervical lymphedema, mother with connective tissue disease or diabetes, congenital heart disease family history and abnormal amniotic fluid are all high risk factors for fetal congenital heart disease.
