Abstract:Objective To assess the result that the use of pit and fissure sealant in the first permanent molars of children aged 9-11 in Xuzhou City, to explore the influencing factor of first permanent molars caries and provide strategies to further improve the effect of pit and fissure sealant so as to provide strategies for further improving the quality of the pit and fissure closure prevention project in Xuzhou City. Methods A total of 518 third-grade school-age children were randomly sampled from 9-11 years old in five project areas of Xuzhou City, and divided into an intervention group (n=261) and a blank control group (n=257) according to whether or not pit and fissure closure was performed. The first permanent molars of the two groups were examined regularly for caries. The intervention group also checked the retention of sealant and filled in the review record form. Questionnaires were sent to parents of school-age children in the two groups. A total of 518 students were once again divided into a caries group (n=54) and a control group (n=464) according to whether they had caries or not, and Logistic regression analysis was performed on the relevant factors that may affect the occurrence of caries in the first permanent molars combined with the recovered questionnaire. Results After one year of pit and fissure sealing, the retention rate of sealant in the intervention group was 88.23%, among which the retention rate of maxillary first permanent molars was 85.19%, the retention rate of mandibular first permanent molars was 91.01%, and the retention rate of mandibular first permanent molars was higher than that of the maxilla, and there were statistical differences between the two groups (Z=-3.512, P<0.001). After one year, the caries ratein the intervention group was 4.2%, and the blank control group was 16.7%, and there were statistical differences between the two groups (χ2=21.726, P<0.05). Logistic regression analysis showed that age (OR=13.223), the frequency of dietary desserts, and carbonated drinks ≥ once a day (OR=2.325) were risk factors for first permanent molars caries in school-age children. Conclusions The implementation of the children’s pit and fissure closure prevention project in Xuzhou City has a good effect on preventing caries in the first permanent molar and reducing the occurrence of dental caries. However, there is still a gap between the oral health knowledge awareness rate of school-age children and the oral health behavior habits and goals, and continuing to do a good job in oral preventive health care education to enable school-age children to develop good oral hygiene habits and improve the attention of caregivers to children’s oral health care is a key problem to be solved in the children’s pit and fusion closure prevention project of Xuzhou City.
